Your Navigation Team can refer you to one of our full-time Financial Counselors for assistance with your insurance questions and information on what insurance plans are accepted by MUSC.

Travel & Lodging

For your convenience, please feel free to fill out the forms below and bring them with you to your first visit. Paper copies will also be available at the registration desk when you arrive.

Medical Records

With your permission, your care team will need to obtain all necessary medical records and test results in preparation for your first appointment. They will also advise you if any additional tests are needed. By gathering this information in advance, your first appointment will be as productive as possible.

In some instances, we may need you to sign a release form for your previous medical records. Your care team will instruct you further if this is necessary.
